CASE MANAGER- Birmingham, AL

Aletheia House is looking for an individual to provide behavioral health services to adult men who have been diagnosed with a substance use disorder, including an opioid use disorder.  Area/County Served:  Jefferson County and surrounding areas.

Location of Position:  Birmingham, Alabama

 

Position Summary:
The case manager will provide supportive services to adult men who have substance use disorders and who are enrolled in the Men’s Hope program. The case manager works closely with the treatment coordinator to accomplish the mission and service goals of Aletheia House.

Primary Responsibilities:

-Help to link and transport clients who are participants in the Men’s Hope program to medical appointments, social service appointments, grocery shopping, etc.

-Help clients apply for supportive services such as food stamps, rental assistance, utility assistance, etc.

-Teach basic living skills classes

-Help clients transition to transitional housing, recovery housing and/or permanent housing

-Document the services provided in the prescribed manner

-Submit documentation, including billing documentation, in a timely and accurate manner

-Perform similar other duties assigned by the treatment coordinator

 

Qualifications:

-Bachelor’s Degree in behavioral health, social services or relevant experience required

-Experience providing supportive services a plus

-Must be able to be certified as a case manager by the Alabama Department of Mental Health within 5 days of employment

-Must have a current Driver’s License and a good driving record

-Motor Vehicle Report required

 

Travel:

-Travel may be required daily or on occasional basis

-Must be willing to use and have your own personal transportation

-Mileage reimbursement for personal use of vehicle is included

-When needed, will drive the agency’s van

 

Job Salary:

$36,000 to $45,000

 

Hours:

Full-time Exempt – Monday-Friday, 8:30 a.m. – 4:30 p.m. – 40 Hours
