Case Manager -Montgomery & Opelika (#0448)

Aletheia House is looking for two individuals who want to make a difference in the lives of
individuals recovering from substance abuse. The areas serviced are Chambers, Lee, Macon,
Montgomery, and Russell counties.

Location of Positions: Montgomery, Alabama and Opelika, Alabama
Office Locations: Montgomery, Alabama and Opelika, Alabama

Position Summary:
The case manager will provide supportive services to individuals with HIV. The case manager
works closely with the HIV services coordinator to accomplish the mission and service goals of
Aletheia House.

Primary Responsibilities:
-To provide mentoring and/or coaching
-To provide recovery resources connecting
-To facilitate and lead support groups as needed
-To build positive relationships within the community
-Assist the therapist in helping program participants find needed community resources
-Help to identify housing units that can be leased by Aletheia House for program participants
-Help clients transition to permanent housing
-Help clients apply for supportive services such as food stamps, rental assistance, utility
assistance, etc.
-Help to secure the furniture, supplies, food and other similar items needed for the supportive
housing program
-Help to link and transport program participants to medical appointments, social service
appointments, grocery shopping, etc.
-To serve as a liaison between participants and connecting with other community organizations
and resources
-To assist program participants with improving job skills
-Provide on-site supervision of program and participants in the evenings
-Document the services provided in the prescribed manner
-Submit documentation, including billing documentation, in a timely and accurate manner.
-Perform similar other duties assigned by the HIV services coordinator, director of behavioral
health, chief operating officer and/or executive director.

Qualifications:
-Bachelor’s Degree in behavioral health or social services required
-Experience providing supportive services a plus
-Must have a current Driver’s License and a good driving record
-Motor Vehicle Report required

Travel Requirements:
-Daily travel is required
-Must be willing to use and have your own personal transportation
-Mileage reimbursement for personal use of vehicle is included

Job Salary:
$34,000.00 – $40,000.00/Annually

Hours:
F/T Exempt – Monday-Friday – 12:00 pm – 8:00 pm – Total Hours – 40
