Aletheia House is looking for an individual who wants to make a difference in the lives of individuals dealing with substance abuse.
The Case Manager is responsible for providing supportive services to program participants. The Case Manager works closely with the Treatment Coordinator to accomplish the mission and service goals of Aletheia House.
Location of Position:
-Assist with MAT inductions.
-Assist with linking program participants to linkages in community services and resources.
-Provide case management by the development of plans that address and identify issues.
-Administer GRPA documentation to new MAT clients.
-Collect GRPA data on new and follow-up MAT clients.
-Serve as an advocate for clients and participants with other service providers.
-Document the history of all services provided to clients in a timely, complete, and accurate manner. Provide summaries and reports as required.
-Attend staff meetings and trainings as required.
-Perform other duties as assigned by the Treatment Coordinator, Director of Behavioral Health, Clinical Director, Chief Operating Officer and/or Executive Director.
-Bachelor’s degree in a social service field or behavioral health required.
-Must be able to complete Case Management Training upon hire.
-Valid Driver’s License and good driving record required.
-Travel is required weekly.
-Must be willing to use and have your own personal transportation. Mileage reimbursement for personal use of vehicle is included.
$15.00 – $20.00 per hour
P/T, Non-Exempt, Tuesday 9:00 am – 5:00 pm—8 hours per week
