HOUSING CASE MANAGER -Birmingham

Aletheia House is looking for individuals to facilitate case management services for individuals, who have a substance use disorder, including an opioid use disorder, and individuals living with HIV and/or other comorbidities.
Office Location of Position:
3720 4th Avenue South, Birmingham, AL 35022
Primary Responsibilities:
-The housing case manager is responsible for assisting in the development of case plans that will address identified issues in the areas of unsubsidized housing, employment, and other core life functions
-Facilitate monthly client meetings, which focuses on enhancing the client basic living skills and empower clients to become self-sufficient
-Assist program participants in finding and maintaining housing
-Help clients and participants obtain appropriate mainstream resources including, but not limited to food stamps, employment, medical, and dental services
-Maintain client files
-Serve as an advocate for clients and participants with other service providers
-Document the history of all services provided to clients in a timely, complete, and accurate manner
-Provide summaries and reports as required
-Attend staff meetings and trainings as required
-Perform other duties as assigned by the treatment coordinator, chief operating officer and/or executive director
Qualifications:
-Bachelor’s degree in a social service field required
-Experience working with individuals who are experiencing homelessness preferred
Travel:
-Travel to program participants’ homes frequently to conduct home visits
-Must be willing to use and have your own personal transportation
-Mileage reimbursement for personal use of vehicle is included
Job Salary:
$36,000 – $45,000
Hours:
0543-A: Full-time, Exempt–Monday – Friday 8:00 a.m. – 4:00 p.m. – Total 40 Hours
0543-B: Full-time, Exempt–Monday – Friday 9:00 a.m. – 5:00 p.m. – Total 40 Hours
Hours may vary depending on client needs.
