Aletheia House is looking for a qualified individual to provide behavioral health services to
individuals who have been diagnosed with a substance use disorder, including an opioid use
disorder, and should have a background in human services to work with recipients of services at
the Aletheia House Bessemer Health Center.
Location of Position: 2201 Arlington Avenue, Bessemer, Alabama 35020
Position Summary:
The Aletheia House Bessemer Health Center’s social worker-case manager is responsible to
provide case management services to individuals who seek services at the health center. The
case manager-social worker works closely with the health services coordinator.
Primary Responsibilities:
-Work with health center service recipients to identify their case management/supportive service
needs and develop a case management plan
-Provide case management services that link health center service recipients to appropriate
community resources
-Serve as an advocate for health center service recipients to access community resources.
When needed, this includes transporting the recipient to the community resources and helping
them apply for services.
-Identify additional community resources that can be used to address recipient needs
-Assess the needs and conduct screenings for appropriateness of MAT services
-Document services provided in a timely and accurate manner
-Attend staff meetings and in-service trainings as directed
-Perform other duties as directed by health services coordinator
Qualifications:
-Bachelor’s Degree in behavioral health or social services required
-Experience providing supportive services and knowledge of community resources a plus
-Licensure or certification as a social worker preferred
Travel Requirements:
-Daily travel is required
-Must be willing to use and have your own personal transportation
-Motor Vehicle Report required
-Must have a current Driver’s License and a good driving record
-Mileage reimbursement for personal use of vehicle is included
Job Salary:
$36,000.00 – $45,000.00
Hours:
F/T Exempt – Monday – Friday, 8:30 am – 4:30 pm
