Case Manager, Opelika, P/T (#0358)

Aletheia House is looking for individuals who want to make a difference in the lives of individuals recovering from substance abuse.  The county serviced will be Lee and Russell County.

 

Location of Position:  Opelika, Alabama

 

Position Summary:
The Case Manager will provide supportive services to individuals with HIV.  The Case Manager works closely with the HIV Services Coordinator to accomplish the mission and service goals of Aletheia House.

 

Primary Responsibilities:

-Help to identify housing units that can be leased by Aletheia House for program participants.

-Help to secure the furniture, supplies, food and other similar items needed for the supportive housing program.

-Help to link and transport program participants to medical appointments, social service appointments, grocery shopping, etc.

-Provide on-site supervision of program and participants in the evenings.

-Help clients apply for supportive services such as food stamps, rental assistance, utility assistance, etc.

-Helps clients transition to permanent housing.

-Document the services provided in the prescribed manner.

-Submit documentation, including billing documentation, in a timely and accurate manner.

-Perform similar other duties assigned by the HIV Services Coordinator, Director of Behavioral Health, Chief Operating Officer and/or Executive Director.

 

Qualifications:
-Bachelor’s Degree in behavioral health or social services required

-Experience providing supportive services a plus

-Must have a current Driver’s License and a good driving record

 

Travel Requirements:

-Daily travel is required.

-Must be willing to use and have your own personal transportation.

-Motor Vehicle Report required.

-Mileage reimbursement for personal use of vehicle is included.

 

Job Salary:

$16.00 – $18.50 per hour

 

Hours:

P/T Non-Exempt – Saturday and Sunday 1:00 pm – 9:00 pm; Total Hours – 16

*May vary according to client needs.
