Outreach/Case Manager –F/T Selma, AL (#0335)

Aletheia House is looking for individuals who want to make a difference in the lives of individuals recovering from substance abuse.  The county serviced will be Dallas County and surrounding areas.

Office Location:  Selma, AL

Position Summary:
The outreach/case manager will conduct recruitment, engagement, retention and follow-up services with judicial and law enforcement agencies and clients who are court-involved.  The outreach/case manager will determine case management needs and complete intake GPRA tool and follow up. The outreach/case manager works closely with the Treatment Coordinator to accomplish the mission and service goals of Aletheia House.

Primary Responsibilities:

-Conduct recruitment, engagement, retention, and follow-up services with law enforcement and judicial agencies and clients who are involved in the criminal justice system.

-Administer the intake and follow-up GPRA to court-involved clients.

-Assist clients in obtaining appropriate mainstream services.

-Document all direct and indirect services in a timely and accurate manner.

-Provide summaries and reports as required.
-To document the delivery of services in a timely and professional manner in accordance with the policies of Aletheia House and the Department of Mental Health.
-Attend staff meetings and in-service trainings as directed.
-Perform other duties as assigned by the Treatment Coordinator, Director of Behavioral Health, Chief Operating Officer and/or Executive Director.

 

Qualifications:

-Bachelor’s Degree in behavioral health or social services required.

-Ability to obtain case manager certification required.

-Must be eligible to provide Medicaid services.  QPP level required.

-Experience providing supportive services a plus.

 

Travel Requirements:

-Daily travel is required.

-Must be willing to use and have your own personal transportation.

-Must have a current Driver’s License and good driving record.

-Mileage reimbursement for personal use of vehicle is included.

 

Job Salary:

$35,000.00 – $37,000.00/Annually

 

Hours: F/T Exempt

Monday, Wednesday, and Friday – 8:00 am – 4:00 pm and Tuesday and Thursday 9:00 am – 5:00 pm

Total Hours – 40
