Aletheia House is looking for individuals to provide linkage and advocacy service to individuals who are HIV positive. The area serviced is Jefferson County and surrounding areas.
Position Summary:
The senior case manager is responsible for providing supportive and on-call services to clients who are referred and enrolled at the 1917 clinic. The senior case manager works closely with the HIV services manager and the director of behavioral health to accomplish the mission and service goals of Aletheia House.

Primary Responsibilities:
-Serve as the initial contact between Aletheia House and the 1917 referral sources
-Help clients and participants obtain appropriate mainstream resources including, but not limited to food stamps, employment, medical, and dental services
-Be available for referrals during evenings and on weekends
-Conduct intakes with all referrals no later than two hours during regular business hours and within one business day after regular business hours
-Ensure housing stabilization, maximum levels of self-sufficiency, and overall better quality of life by maintaining frequent contact with program participants
-Provide case management by the development of plans that address and identify issues in the areas of housing, employment, and other life functions.
-Maintain client files
-Serve as an advocate for clients and participants with other service providers
-Document the history and delivery of all services in a timely, complete, and accurate manner
-Provide summaries and reports as required
-Attend staff meetings and trainings as required
-Perform other duties as assigned by the HIV services manager, director of behavioral health, chief operating officer, and/or executive director
Qualifications:
-Bachelor’s degree in a social service field required
-Experience working with individuals with HIV and who is experiencing homelessness is preferred
-Case management services required with a minimum of three years as a case manager preferred
-Case management certification required
-Valid Driver’s License and good driving record required
-Motor Vehicle Report required
Travel:
-Travel to program participants’ homes frequently to conduct home visits.
-Must be willing to use and have your own personal transportation.
-Mileage reimbursement for personal use of vehicle is included.
Job Salary:
$39,000 – $42,000 DOE
Hours:
F/T, Exempt–Monday – Friday 7:00 am – 3:00 pm – Total 40 Hours
(Hours may vary depending on client needs – available for evenings, weekends, and on call).
